How to Request Equipment Delivery

Make your request at least 24 hours prior to the time you need it
Complete the Equipment Delivery Request Form. Call 778-5004 to confirm all requests. Be sure to include:
Your name
Equipment needed
Day, date, and time class begins and ends
Your phone number, building, and room number
Equipment Pick-Up Service
The following equipment is available for faculty pick up:
- Any of the equipment listed above for delivery service
