Immunization Info

Measles, Mumps and Rubella (MMR) Documentation

According to NYS Public Health Law, all college students born on or after January 1, 1957 who register for 6.0 or more credits are required to provide documentation of immunization against Measles, Mumps and Rubella (MMR.)

Copies of your immunization record may be kept by your high school, doctor's office, military records, or personal immunization booklet. Please include your complete name, date of birth, and social security number on all pieces of information.

This documentation can be submitted to the Student Health Services Office in any of the following ways:

  • Mail a copy of the documentation to:

  • Fax a copy of the documentation to (607) 778-5530
  • Deliver a copy of the documentation to the Student Health Services Office in Science Building, Room 102 between 8:30 a.m. - 4:30 p.m., Monday-Friday.

Meningitis Vaccine Response Form

Meningitis vaccine is NOT required, however the New York State Public Health Law #2167 requires that all college and university students enrolled for at least 6 semester hours or the equivalent per semester, or at least four semester hours per quarter, are required to complete and return the Meningitis Vaccine Response Form. The form must be signed by your school Health Office or your physician.

The above form should be submitted to the Student Health Services Office. It is advised to have the above form submitted to the Student Health Services Office before registering for classes. Failure to have this form on file with the Student Health Services Office prior to the start of classes may result in deregistration from courses.
